A county government had faced years of rising health care costs. This was putting the county in a difficult position. Healthcare benefits are a large % of their annual budget. The county human resources leaders said “We can’t sell more widgets and we can’t raise prices or taxes. So when healthcare costs went up we had to consider painful cuts that would reduce services or put more pressure on our associates. We needed to find a better way forward.”
With the support of a capable consultant the county leadership decided to build have on an on-site primary care clinic. They reviewed the options and chose Activate Healthcare. Activate Healthcare created a “Wellness Council” that helped optimize the design of clinic and created a roll out program to announce the clinic. The clinic was first introduced to every supervisor in a group meeting. Then every county associate was given a presentation in a small group with their supervisor. There was also direct outreach via mail and other means to spouses.
After the clinic was launched there is regular outreach by department encourage members to complete annual physicals. The Wellness Council also helped coordinate health education sessions led by the physician and has continued to meet quarterly for many years to promote a culture of health. The county hired a well-respected local physician, nurse practitioner and two medical assistants. The medical team, Activate leadership and county government meet monthly to discuss the clinic operations and provide continuous improvement.
The clinic began with about 1000 members. Over time other organizations in the community joined the clinic so it now serves over 2000 members in 2 locations. This lowered the cost of the services offered to the county.

The county has been with Activate for over five years, all with the same physician. Participation has averaged over 90% for county employees and over 60% for spouses. Culture survey results showed a significant change in health-related behavior related to diet, exercise and smoking. The key average biometric results improve double digit percentage points in all major categories. Key measures of chronic disease management have improved. The county has saved over $3 million versus their expected trend. For every $1.00 invested in Activate Healthcare, the organization has received back $2.30.
