How to Schedule and Prepare for an Annual Physical

The annual physical exam can play a critically important role in managing your health. This exam allows you to understand potential risks early and avoid the serious consequences that can occur when serious conditions sneak up on you. Your annual physical can also help you build a plan to live a more vital, healthy and active life.

Before your physical exam, you will be asked in for a blood test. Some individuals will receive the results from their blood tests right away, while others will receive their results in a few days. Once you've received your results, you will need to complete an online Health Risk Assessment at http://my.healthmedia.com/. This assessment helps the physician get a more complete picture of your health with questions that assess your height/weight, strength, nutrition, history of injuries, alcohol/tobacco use, and other considerations. Some of the results from your blood tests (e.g. cholesterol and glucose levels) will also need be entered into your online assessment form. Once you've completed your form, click SAVE AND CONTINUE, then rate the confidence and motivation for each question. Click SAVE AND CONTINUE again, then click SUBMIT to complete the process.

If you don't have a computer or you would like assistance completing your form, you can complete your online Health Risk Assessment at the Center before your physical appointment. Please come 30 minutes prior to your scheduled appointment time to allow ample time to complete your Health Risk Assessment.
